Please do not send sensitive health data through the form. Any medical information is discussed confidentially during your visit.

- Responding to your requests and scheduling appointments - on the basis of your consent and of the legitimate interest in serving you.

Form data is kept for as long as is required to handle your request and, if a visit follows, in accordance with the legal obligations for keeping medical records.
